Ascher's or Laffer-Ascher syndrome is characterized by a triad of double upper lip, blepharochalasis, and a nontoxic thyroid enlargement.
Ascher's or Laffer-Ascher syndrome was first described in 1920 by Ascher.1 The disease can be inherited but majority are idiopathic in nature.2 It characteristically presents as a triad of double upper lip, blepharochalasis, and a nontoxic thyroid enlargement is present in this disorder.3 Many conditions can simulate this disorder including hereditary angioedema, early dermatochalasis, acquired cutis laxa and variants of granulomatous cheilitis.4 There are no published case reports of Ascher's syndrome from Nepal till date.
As double lip could be associated with blepharochalasis and non-toxic thyroid enlargement, the same was ruled out by a complete clinical examination.
The acquired form of double lip may be secondary to trauma and oral habit, and may develop in association with Ascher's syndrome which consists of the triad of blepharochalasis, nontoxic thyroid enlargement and double lip.
Superior eyelid entropium and blepharochalasis were seen in one patient during followup.
Conditions may make an upper eyelid appear low (pseudoptosis), including a hypertropia on the contralateral side, microphthalmos, blepharochalasis, phthisis bulbi, dermatochala-sis, or a superior sulcus defect secondary to trauma or cicatrix.
